Triangle Pacific Corp., a kitchen and bath cabinet manufacturer located in Auburn, Neb., was cited for alleged safety violations which carried a proposed penalty of $125,500, according to OSHA.

An inspection was initiated in February after an employee of the company was crushed between two vehicles while attempting to pull a stuck semi-trailer with a powered industrial truck on a slippery, ice-covered incline.

The inspection resulted in one willful, 15 serious and two other-than-serious alleged violations.

The alleged willful violation is for the lack of powered industrial truck inspections, and operating a powered industrial truck with safety defects which required it to be removed from service.

The alleged serious violations include lockout/tagout; powered industrial truck training; machine guarding; woodworking machinery; housekeeping and electrical issues.

The alleged other-than serious violations are for having no handholds on dock plates and general housekeeping in the facility.

Triangle Pacific has 15 working days from receipt of the citations and proposed penalties to comply with them, hold an informal conference with the OSHA area director or contest them before the independent Occupational Safety and Health Review Commission.
