Mississippi-based Southern Tire Mart has racked up 17 health and safety violations following a fatality at its Fort Worth, Texas facility.

An employee who was attempting to mount a monster truck tire rim suffered fatal injuries during the retread process.

“Tragedies like this can be prevented if an employer had followed required procedures for servicing and repairing trucks,” said Timothy Minor, OSHA area director in a statement. “Employers must ensure that workers are trained, and can demonstrate and maintain the ability to service rim wheels safely.”

Agency investigators discovered Southern Tire Mart exposed its workers to struck-by, tire explosion, fire and smoke hazards. In addition, they failed to provide required restraining devices or barriers and implement lockout/tagout procedure

Southern Tire Mart's proposed penalties total $341,195. The company has 15 business days from the date of the citation to respond to OSHA.
