First Source Worldwide LLC Faces $256,088 in OSHA Fines

First Source Worldwide LLC, a Georgia-based chemicals and colorants manufacturer, has been cited for numerous willful and serious workplace safety hazards.

The company racked up $256,088 in proposed fines after OSHA issued several citations after a recent inspection.

“The hazards identified put workers at risk of serious injury,” said Christi Griffin, OSHA Atlanta area office director, in a statement. “Employers must follow required safety practices to protect worker safety and health.”

Following an inspection of the company’s Dalton facility, OSHA discovered First Source Worldwide willfully failed to install a fall protection system as well as develop and implement a written permit-required confined-space program.

The agency also cited the company for several serious and other-than-serious violations, including failing to develop safety procedures when performing equipment maintenance and servicing; failing to train and evaluate forklift operators; failing to ensure employees have proper personal protective equipment; and failing to install machine guards on equipment.

First Source Worldwide has 15 business days to comply, request an informal conference with OSHA’s area director, or contest the findings before the independent Occupational Safety and Health Review Commission.
